Chronic Care at Home at Sharan
This service is intended for patients with chronic conditions during periods of medical deterioration, when they require closer monitoring and more intensive care than what is typically provided by a family physician or the health fund’s home care unit. The goal of the service is to stabilize the patient’s medical condition and restore optimal functioning, all within the familiar, safe, and supportive home environment. The service is provided for a limited period of several months.
Home visits by a nurse at least once a week
- Regular home visits by a physician and a social worker, as needed
- Development of a personalized care plan
- Clinical guidance and consultation for the care team by the company’s medical management and specialists from various medical fields
- Blood and urine tests performed at home
- Referrals for tests and consultations, and issuance of prescriptions for medications and medical equipment
- Guidance and training for the patient and family on effective coping with the illness
- Emotional support and assistance with benefits
- Assistance with administrative or medical-related tasks
Home-based care is fully funded by the HMO’s and involves no cost for patients who meet the eligibility criteria set by the Ministry of Health. The service is provided nationwide.

Service Features
The service is available to adults aged 18 and over and requires the presence of a primary caregiver.
In most cases, the service is provided for a period of 1–3 months.
At the end of the service period, the patient’s condition is reviewed and a decision is made regarding next steps:
If there has been significant relief of symptoms, the patient returns to the care of their family physician or a home care unit within their Kupat Holim.
If there has been no meaningful improvement or if the condition has worsened, referral to Hospice at Home may be considered.
