For patients in the final stage of life, Hospice at Home offers a compassionate and dignified alternative to prolonged hospitalization or institutional care.
The service provides comprehensive medical care at home, in a familiar and meaningful environment, with the patient surrounded by loved ones and supported by a multidisciplinary team of physicians, nurses, and social workers.
At no cost with a referral from Kupat Holim. The service is also available on a private, self-pay basis.

Hospice at Home at Sharan
Within up to 48 hours of referral approval by the HMO’s, an initial intake visit is conducted, including a medical assessment and the development of a personalized care plan.
Nature of care and medical team
Hospice at Home care at Sharan is provided by a multidisciplinary team that includes a physician, a nurse, and a social worker. Care focuses on pain and symptom relief, improving quality of life, and providing emotional support to both the patient and their family.
Visit frequency and team availability
The nurse visits the patient at least once a week and is available 24/7 by phone and WhatsApp. The physician and social workers conduct regular visits as needed. Sharan’s call center is available 24/7, year-round.
Team training and professional consultations
Sharan’s medical teams are trained in palliative care and have extensive experience treating complex medical conditions. They are supported by Dr. Yael Orion, Medical Director, and by consultants with advanced expertise across a wide range of specialties.

Who is the service intended for?
The service is intended for patients with the following conditions in advanced stages:
- Oncological diseases
- Chronic illnesses, including heart, kidney, lung, and liver failure
- Severe stroke
- Dementia and Alzheimer’s disease
- Advanced degenerative neurological diseases, such as ALS, Parkinson’s disease, and multiple sclerosis
- Any other life-threatening illness in an advanced stage
The service is provided to adults aged 18 and over and requires the presence of a primary caregiver.
Components of palliative care at home
Improving quality of life through the management of physical and emotional symptoms, such as pain, shortness of breath, anxiety, depression, and sleep disturbances.
Support in medical decision-making, in collaboration with the patient and family members, while respecting their wishes.
Assistance with benefits and coordination with Kupat Holim, National Insurance Institute, and other relevant bodies.
Emotional support for the patient and their family, including help coping with end-of-life issues, the process of farewell, and finding closure.
